How endometriosis is linked to neurodivergence 

Endometriosis is far from “just a reproductive disorder”. It creates systemic inflammation that affects the whole body, and lesions can persist anywhere. As the picture widens, so does the list of conditions that travel alongside it. Two of the most unexpected are forms of neurodivergence: ADHD and autism.

In this article, we’ll look into how neurodivergence and endometriosis connect and the potential mechanisms behind it - including the shared biological pathways that may connect them.

Endometriosis and ADHD

Women with endometriosis have significantly higher rates of being diagnosed with ADHD, compared to those without endometriosis. In a study following over 850,000 women, those with endometriosis had a 70% higher likelihood of ADHD diagnosis, after adjusting for confounders (1).

And interestingly, when comparing women with endometriosis directly to their own sisters without the endometriosis, their likelihood of ADHD nearly doubled - 98% higher. This suggests the link isn't just explained by family background or genetics, pointing instead to a real, underlying connection between something endometriosis-specific and ADHD.

Researchers found it goes the other way too! Prior ADHD diagnosis was associated with an 81% higher likelihood of being diagnosed with endometriosis later. The bi-direction strengthens the validity of this connection.

Possible Pathways: Why would endometriosis and ADHD be linked?

Researchers hypothesise several theories about shared underlying biological pathways:

Immune dysregulation and neuroinflammation?
Endometriosis drives systemic inflammation, and this doesn’t just affect the body, it impacts the brain and how the brain functions (1). ADHD is being connected more and more to neuroinflammation, which makes this a potential link (2,3).

Shared genetic risk?
Both conditions are known to run in families. Women without endometriosis themselves, but who have a sister with endometriosis, are still 21% more likely to be diagnosed with ADHD than women whose sisters don't have endometriosis (1). That's real evidence of shared familial vulnerability, likely a mix of genetics and shared environment, running through both conditions. But it's notably smaller than the 70-98% increased risk seen in women who actually have endometriosis themselves, suggesting genetics only tells part of the story. Something about having endometriosis specifically appears to add extra risk on top of whatever runs in the family. 

Prenatal and early-life environmental exposure?
One theory put forward in the paper is that prenatal exposure to certain toxins in the environment, dioxins among them, can disrupt developing nervous, endocrine and immune systems (1). That disruption has been proposed as a shared risk factor for both conditions. If true, this would mean endo and ADHD are two downstream effects of the same early exposure. This one remains a hypothesis based on animal studies.

The hypermobility thread?
Endometriosis has also been linked to joint hypermobility (4).This is thought to be due to molecular factors influencing collagen synthesis, breakdown, and remodeling processes - affecting connective tissue structure throughout the body. And separately, hypermobility and related conditions like Ehlers-Danlos syndrome have consistently been linked to higher rates of neurodivergence (ADHD and autism), likely because connective tissue differences also extend to the brain and impact how it functions (5,6). Could a shared connective tissue mechanism be the common thread linking endometriosis and neurodivergence?

It could be that any, none, or all of these could play a role, for different people, in different ways. They are theories, and the mechanisms remain to be confirmed in research.

Endometriosis and Autism

The autism picture is thinner, only because far less research exists. What there is points the same way. In another study in 2,305 adults, there was an elevated likelihood  of endometriosis in autistic females. Autistic females had 2 times the rate of having endometriosis (7). Again, family history didn't explain away the connection.

While research is still unclear about why there might be a higher chance of having endometriosis in autistic individuals. However, the same study also found autistic women were significantly more likely to report hypermobility and Ehlers-Danlos syndrome - tying back to the connective tissue thread we mentioned earlier. 

Researchers have proposed that connective tissue differences, immune involvement, or shared developmental pathways could underlie multiple co-occurring conditions in autistic people, rather than any one condition causing another.

Neurodivergence makes endometriosis harder to manage, and vice versa

The overlap is not only about diagnosis. It reshapes the daily experience of living with endometriosis.

ADHD leaves you more vulnerable to pain, anxiety and low mood (8). 

There is a quiet and cruel overlap in chemistry that can make pain even harder to manage. ADHD is linked to dysregulation of dopamine and noradrenaline. Those are the exact neurotransmitters your body relies on to dampen and control pain (8). If that system already runs differently, pain may be worse. Furthermore, for autistic people, sensory sensitivities can make the pain all consuming.

Dealing with endo pain could also worsen executive dysfunction and focus. Interestingly, one study found that while 7% of the general public has ADHD traits, that rate jumps to over 17% in chronic pain patients. For those suffering from the most severe pain, more than a quarter (27.4%) screen positive for ADHD (8). We wonder whether dealing with pain exacerbates ADHD traits so much that women become more likely to get diagnosed, rather than continue to mask them.

Fatigue is rated in the top 3 most intense symptoms of endometriosis. Having a busy neurodivergent brain can exacerbate this further, and even block you from carrying out what you so desperately want to do.

Managing endometriosis demands precisely what a neurodivergent brain can find hardest: medication schedules, appointments, symptom tracking and follow-ups that hinge on remembering the last one. For someone navigating difficulties with focus, memory, procrastination and time blindness, a treatment routine is not minor admin. It is often the barrier itself.

None of this is a personal failing. It is a mismatch between how care is built for a neurotypical world and how neurodivergent brains work.

The takeaway

Endometriosis and neurodivergence overlap far more than coincidence allows — and that overlap holds up even when researchers control for genetics and family history. The exact mechanisms are still being studied, but the pattern is consistent enough to matter: these conditions can make each other harder to live with, and harder to get properly diagnosed and treated. 

The most honest thing we can tell you is how little research exists on this connection, but that when there is research, the connection is undeniable. That gap is not a footnote, it is the whole point… and it is one we intend to help close!
