The iron-dopamine connection
When you or your child has been diagnosed with ADHD, what follows? Prescriptions, tools and specialists to see. All extremely helpful to manage ADHD moving forward. But what if there was another piece to this puzzle - and it was something far simpler to help reduce those ADHD struggles?
And importantly? This piece is not well known at all, and routinely overlooked by doctors.
A recent meta-analysis found that children with ADHD had 64% higher odds of iron deficiency than those without. And the lower the iron, the more severe the ADHD symptoms. This could be a signal that nutrition may be a far bigger factor in ADHD than we've previously realised - in children, and likely in adults too.
Iron deficiency and ADHD are connected
A meta-analysis pooled data from 17 studies, comparing iron levels in children with and without ADHD.
What did they find?
Children with ADHD had significantly lower ferritin levels, the stored form of iron, compared to controls without ADHD. Ferritin matters here more than serum iron, because it reflects your body's iron reserves, not just what's circulating right now. Those reserves are what the brain draws on.
Crucially, children with iron deficiency showed significantly more severe ADHD symptoms than those who were not iron deficient. The relationship isn't just statistical but it tracks with how difficult things feel day to day.
As you’ll see later, similar is seen in adults with ADHD.
Why does iron matter for ADHD?
If you've read our article on ADHD and the menstrual cycle, you'll know that dopamine is central to the ADHD picture. It governs focus, impulse control, and the ability to initiate tasks.
1. Iron is essential for dopamine production
Iron is an essential cofactor for tyrosine hydroxylase, the enzyme your brain needs to synthesise dopamine.
Without adequate iron stores, that process is disrupted. The result is dopamine dysregulation in the frontal lobe, which is the same dysregulation that defines ADHD.
In a brain already vulnerable to dopamine inefficiency, it can tip the balance in ways that look exactly like ADHD or make existing ADHD considerably worse.

2. Iron, oxidative stress, and neurodevelopment
Secondly, lower ferritin levels are associated with higher oxidative stress, which is suggested to disturb normal neurodevelopmental pathways and further compound ADHD symptoms.
It may does this by:
◉ Disrupting gene expression
◉ Damaging developing neurons
Indeed, ADHD is linked to higher oxidative stress.
Does this apply to adults too?
The studies above were in children, which is where most of the iron-ADHD research has focused so far. But the underlying biology - iron’s role in dopamine production - doesn’t switch off at 18, and there are good reasons to think this connection matters for adults too.
Though limited, the best adult evidence comes from a 2017 study that compared women (aged 18-50) with newly diagnosed, untreated iron deficiency anemia against healthy women (2).
The researchers found that 18.1% of the women with iron deficiency anemia met full diagnostic criteria for adult ADHD, compared with just 2.9% of the healthy controls - roughly six times the rate.
The iron-deficient group also scored significantly higher on retrospective and current ADHD symptom scales, and lower iron and ferritin levels tracked with higher ADHD symptom scores across the group.
Importantly, it's not certain yet why iron deficiency is much more common in ADHD. There needs to be further research into this.
Does iron supplementation actually help?
A recent study tested taking daily iron supplements on 50 children (ages 5 to 15) who were already taking ADHD medication and had low iron levels (3). Half of the children received an iron supplement, and the other half received a fake pill (placebo). Over the 12-week study, parents noticed specific improvements:
Early on (Weeks 1 to 6): The children taking iron had noticeably fewer learning difficulties.
Later on (Weeks 6 to 12): Their behaviour improved in several ways. They also had fewer physical complaints, like tummy aches tied to stress. About 18% of the children also had an increase in their appetite.
The supplement successfully doubled the children's iron levels and most handled it perfectly well.
The Takeaway
Iron isn't a cure for ADHD, and it isn't going to replace the support that many people genuinely need. But the evidence is clear that low iron worsens symptoms, disrupts the exact neurotransmitter system at the centre of ADHD, and is more common in people with the ADHD than it should be.
It's also not known about generally, and routinely overlooked by doctors. This needs to change.

If you or your child has an ADHD diagnosis, asking for ferritin levels to be checked at your next blood test is a reasonable, low-effort step. Not because iron is the answer, but because it might be part of it.
